NAHAPIN (Vision Care)

• $275.00 – 19 years to 64 years every two years.
• $275.00 – 1 year old to 18 years old once a year.
• $275.00 – Diabetics – every two years unless there is a change in the prescription they qualify for a lens change once a year.
• $415.00 – every two years 19 years to 64 years for Clients with a high index prescription of (+/ – 7).
• $415.00 once a year – 1 years old to 18 years old if prescription is high index (+/- 7).
• $415 – every two years – Diabetic (if there is a change in prescription client may qualify for a lens change once a year) If Prescription is high index (+/- 7).
• Clients can use up to $275.00 for glasses, contact lenses, prescription sunglasses (For example – Client can use $175.00 for contacts then they have a balance of $100.00 to spend on glasses or more contacts later on within the two years), Exam is separate from the $275.00 or the $415.00.

NOTE: NEW BENEFITS START EVERY CALENDAR YEAR – JANUARY (for example if an adult aged 19 to 64 years received glasses June 2020 they qualify for new glasses in January 2022).
